Don't select the responsibility

After run the following three requests succefully, in order:
a. "Sync responsibility role data into the WF table."
b. "Synchronize WF LOCAL tables"
c. "Workflow Directory Services User/Role Validation"
with the 'Fix dangling user/roles' and'Add missing user/role assignments' to yes.
found when try to define a new responsiblity,when save ,the system don't allow the saved,
appear the error:'Frm-40735:POST INSERT trigger raised unhandled exception ORA-24033'
